Service Description

Coordinators work closely with participants to develop and implement their NDIS plans, identify and access suitable providers, and coordinate the delivery of supports and services. The role of an SGC NDIS Support Coordinator includes: 1.Helping the participant to understand their NDIS plan and the supports and services available to them 2.Identifying and connecting the participant with service providers that are suitable for their needs and preferences 3.Helping the participant to negotiate with service providers to obtain the best possible services and outcomes 4.Facilitating communication between the participant, their support network, and service providers 5.Providing ongoing support and advice to the participant to ensure that their plan is working effectively for them 6.Helping the participant to review and make changes to their plan as needed Some of our services Include : - Disability support - Services for people with disabilities - Assistive technology - Personalized support - Community participation
